After discharge from the hospital, you may receive a lot of information and it is normal to feel overwhelmed. Talking to your GP is very important.


Tips for discharge:

  • See your GP to get your medication sorted

  • Check your discharge summary and follow instructions

  • Get a referral to see a cardiologist

  • Question if these things don’t seem to line up


Other procedures you may need after heart surgery:

  • Blood tests

  • Echocardiogram (Echo)

  • Ablation

  • Cardioversion

  • Pacemaker

  • Defibrillator


Important things to do:

  • Weight checks to monitor changes

  • Taking medications every day

  • Adhere to fluid restrictions and diet


Tips to manage your condition day-to-day:

  • Consider webster packs (add pic below dot point)

  • Talk to your pharmacist

  • Talk to your GP

  • Breathlessness: Take your time and pace yourself

  • Sleep help: Use pillows to prop yourself up or fans
